OneLondon connects health and care staff to the London Care Record so they have the latest details about a patient when they need it; joining up data from across London in a secure way to support improvements in health and care and giving the citizens of London more access to their health records and more control over their health and care.

The South East London Integrated Care System is a partnership that brings together the organisations responsible for health and care services in south east London, to make the greatest possible contribution to the health and wellbeing of people living in the six boroughs.

Discovery East London

ELHCP

Discovery East London is a clinical partnership programme first established in 2016 to create a linked dataset of real-time health across five boroughs: City of London, Hackney, Newham, Tower Hamlets and Waltham Forest.

Whole Systems Integrated Care (WSIC)

The Whole Systems Integrated Care Dashboards are a suite of tools available to clinicians and care professionals who are providing direct care to patients. The WSIC Dashboards provide a linked integrated summary of patient's health and social care which can be used to case find and case manage patients who require more targeted and proactive care. The Dashboards aim to support clinical staff in improving timeliness and quality of care for patients across NWL London.

We bring together data from Primary Care, Acute, Mental Health and Community Services, to serve the people providing care to our citizens.

Targeted Prostate Cancer Healthcheck

The Targeted Prostate Health Check (TPHC) project supports NHS England’s post-pandemic cancer recovery objectives and also the Long Term Plan cancer ambition to diagnose 75% of all cancers at an earlier stage, by 2028.


NHS 111 Frailty API

"As a London Ambulance Service (LAS) clinician who provides clinical expertise for NHS 111, I want to obtain access to the local information within the Discovery Dataset to determine whether the caller is potentially seriously frail.”


North West and North East London Cerner HIE

Data for GP, Community, and Acute services are being made available to the Cerner HIE via a FHIR API and is being actively used in hospitals to access patient's records at their point of care.


East London Genes and Health

The East London Genes and Health (ELG&H) study aims to improve the health of people of British-Pakistani and Bangladeshi heritage by analysing the genes and health of 100,000 local people.


Childhood Immunisations

The childhood immunisations extract provides the North East London Foundation Trust (NELFT) Child Health Information Service (CHIS) team with child health data from GP systems.


Diabetes retinopathy recall

Homerton University Hospital manages and provides the North East London Diabetes Eye Screening Programme.


Barts Vaccine Safety

This project, using an alerting tool and algorithm, tracked the clinical events of citizens vaccinated against COVID19 who then subsequently attended any of the three emergency departments operated at Barts Health NHS Trust.
